Global population aging represents, nowadays, a key challenge for health care. A challenge that primary care services intend to face. A challenge that will require a community based, multidisciplinary organization, focused on patients with polypathology. This challenge implies the collaboration between medical specialists, nurses and physiotherapists, with the aim to offer personalized health care and support to chronic and polypathological patients.
