DM 77/2022 represents the most recent regulatory intervention in communitybased healthcare. However, its implementation in local contexts has unfolded through different pathways. The main conditioning factors concern available resources and the specific characteristics of territories (institutional, organizational and social). Also, the regulation is closely intertwined with the resources of Pnrr, which have represented an opportunity to expand intermediate care facilities. This paper analyses the process of implementation and reorganization of community healthcare in the province of Modena, adopting a MixedMethod approach that integrates documentary data with qualitative data collected through semistructured interviews with institutional and healthcare decisionmakers, care professionals and social partners. The findings show that DM 77 is a driver of multilevel processes, fostering and deepening dynamics of territorial governance and concertation, as well as shaping everyday work practices.
