The need for a targeted approach to prevent and treat addictions among young people aged 14 to 24 has resulted in the implementation of a collaborative working model at ASST Santi Paolo e Carlo in Milan. The model involves the Territorial Addiction Service's Early Diagnosis and Treatment (DTP) team and the Centro Giovani Ponti (CGP), both of which belong to the Department of Mental Health and Addiction (DSMD). The aim of this partnership is to provide young patients and their families with a welcoming and accessible environment where they can receive support and assistance. The model seeks to reduce "the duration of untreated disease" by facilitating early diagnosis and timely therapy. Specifically, the CGP acts as a gateway to specialized addiction treatment services, making it easier for patients to access them. This article outlines the operating model implemented through this collaboration and reports on the preliminary results achieved between March 2021 and August 2022. The article discusses the specific cases in which the model is applied, the use of diagnostic tests, and the benefits of collaboration between services. Overall, the partnership between the DTP team and CGP has shown promising results in addressing the needs of young people and their families.
