During the course of the years donation rates have notably changed between European countries. Nowadays, in Italy episodes of blood supply shortage represent still a significant challenge. It is important to understand what determines this variation in percentage and how blood offer can be subsidized. As a result of current massive immigration levels, intercultural differences must be taken into consideration. In an increasingly open and multicultural society, we should understand whether values that drive blood donation can be comprehended and shared. We discuss the role of financial and other incentives in eliciting blood donations or undermining pro-social actions. The evidence suggests that creating publicity campaigns or adapting the blood collecting system to the particularities of the target population, providing support and information, will be useful in order to promote willingness to donate. Adequate non-cash incentives related to both individual and local needs must also be implemented.
