The aging of the migrant population in Italy is an increasingly significant phenomenon, challenging the country’s socio-health systems. The steady growth of this demographic group needs a deeper reflection on welfare policies and dedicated services to adequately address the specific needs of elderly migrants, often more vulnerable due to various social, economic, and cultural conditions. The paper analyses the current state of the art on elderly migrants’ health at European level, highlighting the limitations and fragmentation of these policies. It focuses on specific aspects of the National Recovery and Resilience Plan (NRRP), aiming to enhance services for the elderly by promoting a proximity-based welfare system and fostering autonomy in older age. Finally, it underscores the importance of more inclusive policies and participatory strategies, responding to the distinct needs of over sixty-five migrants. Through targeted interventions, equitable access to socio-health services can be ensured, promoting healthy and active aging, while preventing marginalization and facilitating the full integration of this population segment.
