The author analyses the multidimensional crisis of Italian general practice (GP) in the post-pandemic period from a socio-systemic perspective. Drawing on Luhmann’s functional subsystems framework and De Toni’s complexity dilemma, the paper examines the structural tension between specialist differentiation and generalist redundancy in the public health system. The main challenges facing primary care are discussed: the epidemiological transition toward chronic disease, medicalisation, demand induction, prescriptive misalignment between primary and secondary care, vocational crisis, and progressive healthcare desertification in rural areas. The proposed university specialisation in GP is critically assessed as a paradoxical oxymoron. The author instead advocates two bottom-up approaches: the Community of Practice (Wenger) as a tool for professional identity renewal, and Lipsky’s street-level bureaucracy theory as a key for interpreting the clinical discretion of GPs in translating health policies into territorial practices.
Keywords: general practice; polycrisis; complexity; specialist differentiation; community of practice; street-level bureaucracy.
